TRANSFORM MY SCORE SMS / TEXT MESSAGE CONSENT DISCLOSURE

Effective Date: May 1, 2026 Last Updated: May 1, 2026

This disclosure explains how Transform My Score uses SMS (text message) communications to contact you, what your consent means, and how to opt out at any time. By providing your phone number and consenting to receive text messages from Transform My Score, you agree to the terms described below.

This disclosure is part of and incorporated into our Privacy Policy and Terms of Service.

1. WHO WE ARE

Business Name: Hermox Document Solutions LLC Doing Business As: Transform My Score Website: www.TransformMyScore.com Address: 700 W Big Beaver Ste 330, Troy, MI 48084 Contact: [email protected] | 586-787-2909

Transform My Score is a credit repair service. We help consumers identify and dispute potentially inaccurate items on their credit reports. We are not a law firm.

2. WHAT YOU'RE CONSENTING TO

By providing your phone number and indicating consent (through a checkbox, verbal "yes," or affirmative response in chat), you agree to receive SMS messages from Transform My Score about:

Free consultation appointment confirmations and reminders

Service update notifications and dispute progress updates

Follow-up messages regarding your inquiry or case

Information about our credit repair services

Promotional offers and service updates from Transform My Score

3. CONSENT IS NOT REQUIRED FOR PURCHASE

You are not required to consent to SMS messages to purchase Transform My Score services. You can complete a consultation and purchase a credit repair service plan using only email or phone call communication.

Providing SMS consent is entirely voluntary. You can decline SMS consent at any time without affecting your ability to use our services.

4. MESSAGE FREQUENCY

Message frequency varies based on your interactions with us. Typical volume is 1 to 5 messages per month. During an active credit repair engagement, you may receive more frequent messages as dispute results come in. After your case is complete or paused, message volume typically drops to occasional check-ins or service updates.

5. MESSAGE AND DATA RATES

Message and data rates may apply based on your mobile carrier plan. Transform My Score is not responsible for any charges from your wireless carrier. Standard messaging rates from your carrier apply.

6. HOW TO OPT OUT

You can stop receiving SMS messages from Transform My Score at any time. There are several ways to opt out:

6.1 Reply STOP

Reply with any of the following words to any SMS message from us:

8. TRANSACTIONAL MESSAGES AFTER OPT-OUT

If you opt out of marketing SMS but have an active service with Transform My Score (e.g., you've purchased a credit repair plan and your case is being actively worked), we may still send you transactional messages directly related to that active service — such as dispute result notifications, important case updates, or appointment confirmations.

To stop all SMS communications, including transactional, complete your active service with us or email [email protected] to discuss alternative communication methods.

9. WHEN YOU MAY RECEIVE A NEW CONSENT REQUEST

If you opt out of SMS messages and later wish to re-enroll, you can do so by:

Providing your phone number again through a Transform My Score form with the consent checkbox

Replying START to a transactional message you receive

Contacting us directly to request re-enrollment

Each re-enrollment is treated as a fresh consent and recorded in our compliance records.

10. NO THIRD-PARTY SHARING

Transform My Score does not share, sell, rent, or transfer your phone number to third parties for their marketing purposes. Your number is used only by:

Transform My Score and Hermox Document Solutions LLC for the messages you've consented to receive

Our SMS service provider (a third-party platform that powers our text messaging) for the technical delivery of those messages, under contractual obligations to use your data only for that purpose

We will never sell your number. We will never share it with affiliated businesses (including BlockMyDebt and GetPaidDocs) without separate, specific consent from you.
